Fast-Track Referrals are for support coordinators, providers, families, nominees and carers who need a timely first response from a local team.
Once we receive the details, we review the participant’s circumstances, clarify urgency and arrange the Needs Assessment and Support Onboarding conversation if GPM is a suitable fit.
Fast-track is about removing avoidable waiting, not rushing the person behind the referral.

01Who can use Fast-Track Referrals?
Support coordinators, providers, families, nominees and carers can use the Referral Portal to start a new participant referral.
02What services can a referral be for?
Referrals can be for Support Coordination (Level 2), Specialist Support Coordination, or Training for Parents and Carers. Every new referral begins with a Needs Assessment and Support Onboarding conversation.
03What does the 24-hour response mean?
It means GPM will respond to every new referral within 24 hours to acknowledge it and discuss the appropriate next step. It is not a promise that all support will commence within 24 hours.
04What if the participant’s situation is high-risk?
Explain the urgency in the referral. Specialist Support Coordination can help with crisis planning, multiple stakeholders, safeguarding, risk mitigation and barriers to implementing supports.
